By accessing or using the Advanced Family Medicine and Wellness Center website, you agree to be bound by these Terms of Service.

Appointment Policy

We require at least 24 hours notice for appointment cancellations or rescheduling. Missed appointments or late cancellations may be subject to fees.

Payment Terms

Payment is due at the time of service. We accept most major insurance plans—you must have your insurance card physically present at your visit. If you cannot provide proof of insurance, payment will be required at the time of service and we will bill your insurance afterward.

We also welcome patients without insurance and offer cash-pay services.
